Privacy Policy
Effective date: October 21, 2026
Integrated Care Clinicians, LLC respects the privacy of people seeking information about opioid treatment and recovery support. This policy explains how information is handled when you visit iccproviders.com or use its general contact form.
Website Information and Patient Records
This website privacy policy covers website inquiries and general online information. It is not a complete Notice of Privacy Practices for clinical care. Health records may be subject to HIPAA, Michigan law, and the additional confidentiality protections for certain substance use disorder records under 42 CFR Part 2. Please call our office to request the applicable patient privacy notice, discuss a medical-record request, or ask how these protections apply to your care.
Information You Choose to Provide
Our contact form asks for your first and last name, email address, and message. A phone number is optional. We receive the information you submit so that your inquiry can be reviewed and answered. Please provide only the details needed for scheduling or a general question.
Do not enter medical records, diagnoses, medication details, insurance identification numbers, Social Security numbers, payment-card information, or other sensitive health information in the website form. The form is a general inquiry channel, not a secure patient portal or a way to request urgent treatment.
How Website Inquiries Are Used
Information submitted through the contact form is used to respond to your question, discuss next steps, and help coordinate a requested appointment. Submitting a form does not confirm an appointment, establish a treatment relationship, or enroll you in a marketing mailing list. Clinical decisions and treatment arrangements require direct communication with the practice.
Inquiries may be handled by the practice and authorized people who help manage the website or respond on its behalf. Information needed to operate the website may also be processed for technical support, reliability, and security. Contact-form information is not offered for sale or supplied to advertisers for their independent marketing.
Formlets and Other Service Providers
The embedded contact form is provided by Formlets. When you submit it, the information passes through that service and may be stored there and delivered by email to the designated inquiry recipients. Website hosting and email providers also help process these communications. You can review the Formlets Privacy Policy for information about its service.
Embedded services, fonts, and links may connect your browser to another provider. Those providers may receive technical information needed to deliver their services. A link to another website does not make its privacy practices the same as ours; review its policy before providing information.
Technical Information and Cookies
When you visit, website servers and embedded services may receive information such as your IP address, browser and device type, requested pages, and the time of a request. This information can help deliver pages, troubleshoot problems, and protect the service. Cookies or similar browser storage may be used by website features or embedded providers.
You can review or restrict cookies through your browser settings. Blocking them may affect the contact form or other features. This policy does not promise that every embedded provider responds to browser Do Not Track signals in the same way.
Security, Retention, and Your Choices
The website uses HTTPS to help protect information while it travels between your browser and the site. No website, email system, or internet transmission can be guaranteed completely secure. Please call the office to arrange an appropriate communication method for sensitive matters.
You may choose to browse without submitting the form. To ask about an inquiry you sent, correct your contact details, request deletion of website inquiry information, or ask us to use a different contact method, call the office. We may need to verify your identity before acting. Some information may need to be retained for legal, recordkeeping, security, or business purposes; medical records follow applicable retention rules.
Health Information Privacy Rights
Where HIPAA applies, patients have rights that can include requesting access to their records, asking for corrections, requesting certain restrictions or confidential communications, receiving information about certain disclosures, and obtaining a privacy notice. Requirements and exceptions depend on the request and applicable law. Please contact the office for the appropriate process; do not send a medical-record request with sensitive details through the general website form. Learn more from HHS about health information privacy rights.
Certain records identifying a person as receiving substance use disorder services have additional protections under 42 CFR Part 2. Depending on the circumstances, use or disclosure requires an appropriate consent or a specific legal exception. These protections also limit use of protected records in legal proceedings against a patient. Our office can explain the notices and consent forms relevant to your care. See HHS information about substance use disorder record confidentiality.
Privacy Questions and Complaints
Call our office and ask to speak with the person responsible for privacy questions if you have a concern. You may also file a health information privacy complaint with the U.S. Department of Health and Human Services Office for Civil Rights if you believe HIPAA or Part 2 rights have been violated. Exercising applicable privacy rights or filing a complaint must not result in retaliation.
Children, Updates, and Contact Information
This website provides general information for people seeking care and their families. Parents or guardians should contact the practice directly about services for a minor rather than have a child submit personal details online.
Updates to this website policy will be posted on this page with a revised effective date. This policy does not reduce any privacy rights provided by applicable law.
